West Virginia Immunization Law & Entry Requirements:

• CS64-95 - Immunizations for School Entry
• West Virginia law pertaining to School Attendance, Health & Safety: WV State Code 18-28-2

Specific immunization entry requirements:

• Day Care  
• Pre-Kindergarten (revised) 
• School Entry K-12 
• 7th Grade & 12th Grade Entry (Effective - School Year 2012-2013)
• College & University                                                     

Note:  West Virginia law does not allow for non-medical exemptions for school entry.
