State Code and Administrative Rule:

CS64-95 - Immunizations for School Entry
West Virginia law pertaining to School Attendance, Health & Safety: WV State Code 18-28-2

Specific immunization entry requirements:

Day Care
Pre-Kindergarten (revised 2013)
School Entry K-12
7th Grade & 12th Grade Entry
College & University

Note: West Virginia law does not allow for non-medical exemptions for school entry.
